Northwest Dawah Foundation General Event Participation Agreement and Liability Waiver
Participation in any events or activities organized by Northwest Dawah Foundation is subject to agreement by the participant to the following terms:
- Waiver and Release: The event participant releases and forever discharges and holds harmless Northwest Dawah Foundation and its staff, volunteers, partners successors, and assigns from any liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from the activities as a participant with the Northwest Dawah Foundation, including claims arising out of negligence. Participant understands and acknowledges that this release discharges Northwest Dawah Foundation from any liability or claim that I may have against Northwest Dawah Foundation concerning bodily injury, personal injury, illness, death, or property damage that may result from the participation in events and activities organized by the Northwest Dawah Foundation.
- Insurance: I affirm that I am covered by primary medical insurance and understand that I am responsible for my medical bills if injury occurs. Further, I understand that Northwest Dawah Foundation does not assume any responsibility for or obligation to provide the participant with financial or other assistance, including but not limited to medical, health, or disability benefits or insurance of any nature in the event of the participant’s injury, illness, death or damage to his or her property. Participant expressly waives any such claim for compensation or liability on the part of Northwest Dawah Foundation beyond what may be offered freely by Northwest Dawah Foundation in the event of such injury or medical expenses incurred by the participant.
- Assumption of Risk: I understand that activities or events organized by Northwest Dawah Foundation include activities that are inherently dangerous to me or have significant risks for me.
- Photographic Release: Participant grants and conveys to Northwest Dawah Foundation all rights, titles, and interests in any photographs, images, video, or audio recordings of the participant or his or her likeness or voice made by Northwest Dawah Foundation in connection with the participation in Northwest Dawah Foundation events, including but not limited to, any royalties, proceeds, or other benefits derived from such photographs or recordings.
- Medical Treatment: Participant releases and forever discharges Northwest Dawah Foundation from any claim whatsoever which arises or may hereafter arise on account of any first-aid treatment or other medical services rendered in connection with an emergency during participation in Northwest Dawah Foundation events or activities and give my consent for the Northwest Dawah Foundation to provide, administer, or obtain medical treatment for me.
- Other: Participant expressly agrees that this release is intended to be as broad and inclusive as permitted by the laws of the State of Oregon and that this release shall be governed by and interpreted by the laws of the State of Oregon. I agree that if any clause or provision of this release is deemed invalid, the enforceability of the remaining provisions of this release shall not be affected.
- Islamic Arbitration: The participant agrees to participate in Islamic arbitration involving a quality Muslim Scholar and in consultation with a local Sunni mosque in resolving any issues arising from participating in activities organized by Northwest Dawah Foundation.
- Minor Participation: Any participant under the age of 18 years of age must have parent or guardian permission to participate and their parent is responsible for supervising their minor child during the event or granting permission for an adult-aged volunteer to act as their temporary chaperone during the event or activity.
